Case Number: CLI-2026-1029
Shipper: Confidential (E-commerce Seller, Shenzhen)
Logistics Provider: Confidential (Freight Forwarder, Ningbo)
Carrier: Confidential (Ocean Carrier, 2M Alliance)
Destination: Amazon FBA Warehouse, Los Angeles (LGB8)
Container Number: MSCU-XXXXX-X
BOL Number: MSC-XXXXX-XX
Date of Loss: August 12-15, 2026
Report Date: September 6, 2026
Prepared By: Asia Logistics Investigations (ALI), Hong Kong
1. Executive Summary
This report summarizes the investigation into the loss of 15 cartons from a single 40-foot container shipment from Shanghai to the Amazon FBA warehouse in Los Angeles (LGB8).
Shipment Summary:
-
Total cartons loaded in Shanghai: 80
-
Total cartons delivered to LGB8: 65
-
Missing cartons: 15
-
Lost inventory value: $14,800 (wholesale)
-
Lost retail value: $38,000 (estimated)
-
Claim filed: $14,800
Key Finding: The 15 cartons were not stolen. They were never loaded onto the container in Shanghai. The "delivered" status in the tracking system was a system-generated event, not a physical delivery confirmation.
Recommendation: The shipper should implement a photo-based loading verification process. The shipper should also require the logistics provider to provide a dock receipt with carton count, not just container count.

| Factor | Description |
|---|---|
| Primary | 15 cartons were staged separately due to damaged packaging and were never loaded |
| Contributing | The warehouse did not conduct a final carton count before sealing the container |

9. Recommendations
9.1 For the Shipper
-
Implement a photo-based loading verification process. Require warehouse staff to take a photo of the loaded container showing the carton count before sealing.
-
Require a dock receipt with carton count. Do not accept a BOL that only shows "1 container." The BOL should show the carton count.
-
Conduct a physical count at the warehouse before the container is sealed. Send a representative to witness the loading, or hire a third-party inspection service.
-
Do not rely on the tracking system's "delivered" status. The status is system-generated and does not confirm carton-level delivery.
-
Review the packing list and BOL for consistency. The packing list and BOL should match. If there is a discrepancy, resolve it before shipment.
9.2 For the Freight Forwarder
-
Implement a two-person verification process for container loading. One person counts, one person verifies the count before sealing.
-
Notify the shipper of any cartons set aside for re-packaging. The shipper should be informed and should approve the removal or re-packaging of any cartons.
-
Update the warehouse training. The warehouse manager should be trained on the importance of carton count verification and the consequences of errors.
-
Provide a dock receipt showing the final carton count, signed by the warehouse manager and the shipper's representative (or a third-party inspector).
10. Claim Assessment
| Claim Element | Amount (USD) |
|---|---|
| Inventory value (wholesale) | $14,800 |
| Shipping cost (pro-rated) | $1,200 |
| Lost retail revenue (estimated) | $38,000 |
| Claimed amount | $14,800 |
Assessment: The claim for the missing 15 cartons is valid. The shipper is entitled to compensation for the lost inventory value.
Recommendation: The freight forwarder has accepted liability for the lost cartons. The freight forwarder's insurance will cover the claim.
11. Conclusion
The loss of 15 cartons from shipment #CLI-2026-1029 was caused by a warehouse error at the freight forwarder's Ningbo facility. The cartons were staged separately for re-packaging but were not loaded into the container. The warehouse did not conduct a final carton count before sealing the container.
The tracking system's "delivered" status was system-generated based on container arrival. It did not confirm carton-level delivery.
Final Status: Case closed. Freight forwarder has accepted liability. Compensation of $14,800 has been agreed. The shipper has implemented the recommended loading verification process.
